Did a Research About The Use of Lippes Loop Intrauterine Device, FMUI Doctor Received Doctoral Degree

Lippes Loop (LL) type Intra Uterine Device (IUD) is a contraceptive tool commonly used in the early development of IUD, because of the low price and can be inserted for a long time. However, LL IUD is easily detached and relatively hard to insert. Because of those factors LL IUD was replaced with Copper T IUD (CuT).

CuT IUD contains polyethylene and copper, and it is smaller in size, it is also easier to insert and not easily detached. However, the copper ingredient of CuT IUD can harm epithelial cells on the endometrium. It can delay the return of fertile period after CuT IUD is detached, especially after long term use. Pelvic inflammatory disease can also happen in CuT IUD user and many CuT IUD’s arms was found broken.

CuT IUD’s benefits are they increase the synthesis of Glycodelin A (GdA), or a protein released by epithelial cells and endometrial cells that inhibits fertilizations. GdA mechanism in inhibiting fertilization is unknown. The use of IUD causes inflammatory process which was marked by the infiltration of macrophage, TNF-a, IL-4 and PGI-2 which are believe to be related to the increase of GdA, therefore GdA is believed to be one of the mechanism of fertilization inhibition by CuT IUD.

LL IUD also causes inflammation, but it is unknown whether or not it causes an increase of GdA. CuT IUD causes inflammation, break down of endometrium, and causes delayed return of fertile period after longterm use. The use of LL IUD probably doesn’t cause endometrial changes, but it hasn’t been researched.

Teacher and researcher of Obstetrics and Gynecology Department FMUI-RSCM, dr. Irvan Adenin, SpOG, did a research to analyze inflammatory components after insertion of polyethylene compared to polyethylene plus copper and its relation to gdA synthesis by endometrium.

This research was done in 22 laboratory Sprague Dawley rats, being grouped into two, one group received CuT IUD (polyethylene and copper), and the other group received LL IUD (polyethylene). And then the rat’s blood and tissue was analysed by enzyme-linked immunoabsorbent assay (ELISA) and immunohistochemistry.

The result of this study shows that GdA expression is higher on the LL IUD group compared to CuT IUD group. Therefore, IUD LL which was abandones many years ago can actually be used again. As a conclusion, this rsearch shows that the funding of IUD can be pressed down to have result that is just as effective.

The research was presented by dr. Irvan Adenin, SpOG on his doctoral inauguration, Thursday, July 11th 2019 in IMERI Auditorium. The dissertation was titled “The Relation between Inflammatory Components with Glycodeline A and its Role in Lippes Loop IUD Mechanism”.

Acted as the head examiner, Prof. Dr. dr. Suhendro, SpPD-KPTI, and other examiner includes Prof. Dr. dr. Soegiharto Soebijanto, SpOG(K); Prof. Dr. dr. Murdani Abdullah, SpPD-KGEH, FINASIM, FAGC; and Prof. Dr. dr. Tono Djuwantono, SpOG(K), M.Kes from Padjajaran University.

To close the session, Prof. Dr. dr. Ari Fahrial Syam, SpPD-KGEH, MMB as the head of the ceremony inaugurate dr. Irvan Adenin, SpOG as a Doctor in Medical Science of FMUI. On their opening speeches Prof. Dr. dr. Wachyu Hadisaputra, SpOG(K) as promotor, and Prof. drh. Arief Boediono, PhD and dr. Nurjati Chairani Siregar, MS, PhD, SpPA(K) as co-promotors hoped that this study can be useful for family planning programs in Indonesia.
